Prescription Refills

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor. You may request a prescription refill by phone as well.

Please note that in compliance with federal regulations, we can only fill prescriptions for our own active patients. To be considered active, a patient must have been examined by our doctor within the past 365 days.

We will notify you via email or phone when your pet's prescription is approved and ready to be picked up or if the prescription was denied and the reason. We will also inform you of the total cost of the prescription. Please indicate your preferred method of contact on the form.

If you would prefer to have the prescription mailed to you, please indicate so in the additional information area. You may provide a credit card number over the phone. A service fee and shipping charges will apply.
